Community Care Services

PAC Ashfield Community Care Services incorporates two programs - Community Aged Care Packages (CACPs) & Day Therapy.

 


Community Aged Care Packages (CACPs)

CACP are tailored to meet individual care needs of people to remain living in the homes the ASHFIELD & DRUMMOYNE LGAs (local government area) are very familiar to the staff, who are able to take CACP recipients to doctors, shopping & outing with ease.

We don't have great distances between CACP recipients and this ensures time efficiency of service.

Currently the CCS staff has members with CALD (culturally and linguistically diverse) backgrounds which allows for multicultural meals & the ability for CALD recipients to access the service even though the CACPs are mainstream.

The services are very flexible and can be changed as needs change.

Care plans are regularly reviewed to make sure the service received continues to be appropriate.

CACPs can be for 7 days a week service & there is an on call number that allows staff & recipients to contact the manager.

Day Therapy Centers

The Day Therapy Centers are a unique program offered by PAC that stimulates & encouraged members to participant in activities of healthy being from Tai Chi to Yoga, Snozolehn therapy, relaxation therapies as well as having guest speakers on topics such as falls prevention & healthy living.

 


 
The groups are small to ensure one on one attention.
 
 

Transport is provided for the Ashfield & Woollahra LGAs.

The Day Therapy Centers have trained natural therapist to provide the program in an individualised format

The Day Therapy Centers are located in the residential facilities, which enable the participants to become familiar with PAC, i.e. its staff, food & facility if the need ever arises for residential care.
